What's Typically Covered by Insurance

Coverage depends on your specific plan and whether the visit is medical or routine vision. Here's how it generally breaks down at Mudgil Eye Associates:

Medical eye exams (covered by medical insurance and Medicare)

  • Diabetic eye exam
  • Glaucoma evaluation and management
  • Cataract evaluation and surgery
  • Eye injury or eye emergency visit
  • Strabismus, amblyopia, and pediatric eye disease evaluation
  • Dry eye treatment when symptomatic
  • Macular degeneration screening and management
  • Eye infection or red eye visit

Routine vision exams (covered by vision insurance)

  • Annual comprehensive eye exam without medical concerns
  • Eyeglasses prescription
  • Contact lens fitting and evaluation
  • Frame and lens benefits (varies by plan)

Many patients have both medical and vision insurance. We bill the appropriate coverage based on the reason for your visit. If your visit starts as routine but a medical issue is found, we may need to bill a portion to medical insurance, and we will explain this to you before charges are processed.

Medicare and Cataract Surgery

Medicare Part B covers medically necessary cataract surgery, including the surgeon fee, the facility fee, the standard monofocal intraocular lens (IOL) implant, and one pair of standard eyeglasses or contact lenses after surgery. Medicare typically pays 80 percent of the Medicare-approved amount and you are responsible for your Part B deductible and 20 percent coinsurance, which is often paid by a secondary or supplemental plan.

What Medicare does not cover

  • Premium IOL upgrades like PanOptix, Vivity, the Light Adjustable Lens, Symfony, and toric IOLs (out-of-pocket)
  • Femtosecond laser-assisted cataract surgery when used for refractive precision (out-of-pocket)
  • Routine vision exams when there is no medical concern
  • Eyeglasses or contacts for refractive correction outside of the one post-cataract pair
  • EVO ICL and other elective vision correction procedures

Dr. Mudgil's office will provide a clear written estimate before scheduling any surgery so you know exactly what Medicare covers, what your supplemental plan covers, and what your remaining out-of-pocket cost will be.

Premium IOL Upgrades and Out-of-Pocket Costs

Cataract surgery with a standard monofocal lens is covered by Medicare and most commercial insurance. Patients who want to reduce their dependence on glasses after surgery can choose to upgrade to a premium IOL. The base cataract surgery is still covered; only the lens upgrade and any laser refractive component is out-of-pocket.

Typical premium IOL out-of-pocket ranges (per eye) at Mudgil Eye Associates are similar to most U.S. cataract practices. Final pricing depends on the specific lens, the use of femtosecond laser, and any additional refractive correction. Dr. Mudgil's team provides a written quote at your consultation.

Premium IOL options at Mudgil Eye Associates

  • Toric IOL for astigmatism correction
  • Vivity extended depth-of-focus lens for distance and intermediate vision
  • PanOptix Trifocal for distance, intermediate, and near vision
  • Odyssey trifocal lens
  • Symfony extended depth-of-focus lens
  • Light Adjustable Lens (LAL) by RxSight, the only IOL adjustable after surgery

Payment and Financing Options

HSA and FSA Accounts

Most elective vision services at Mudgil Eye Associates are HSA-eligible and FSA-eligible expenses, including premium IOL upgrades, femtosecond laser cataract surgery, EVO ICL, and contact lens fittings. Many patients use HSA/FSA dollars to offset out-of-pocket costs for advanced cataract surgery and refractive procedures. Bring your HSA or FSA debit card on the day of payment, or submit a receipt for reimbursement.

CareCredit Financing

CareCredit is a healthcare credit card that offers no-interest and extended financing options for elective procedures including premium IOL cataract surgery and EVO ICL. Many patients spread their out-of-pocket cost over 12, 18, or 24 months. Apply at CareCredit.com before your consultation if you would like to discuss financing.

Cash, Check, Credit Card

We accept cash, personal checks, and all major credit cards (Visa, MasterCard, American Express, Discover). Payment is collected at the time of service for any patient responsibility (copay, deductible, or out-of-pocket amount).

Written Estimates Before Surgery

For any cataract surgery, EVO ICL, or other surgical procedure, our team prepares a written estimate showing what your insurance covers and what your final out-of-pocket cost will be. We never bill you a surprise.

Frequently Asked Questions About Insurance and Cost

Do you accept my insurance?

We accept most major medical and vision insurance plans including Medicare. Plan participation can change, so always call our office at 610-429-3004 before your appointment to verify your specific plan.

What's the difference between medical and vision insurance for an eye exam?

Medical insurance covers eye exams that address a medical concern (cataracts, glaucoma, diabetes, eye pain, injury, infection, or any disease of the eye). Vision insurance covers routine eye exams, glasses prescriptions, and contact lens fittings when there is no medical concern. Many patients have both. We bill the appropriate coverage based on the reason for your visit.

How much does cataract surgery cost out-of-pocket?

For most patients with Medicare or commercial insurance, basic cataract surgery with a standard monofocal IOL is fully covered after your deductible and copay. If you choose a premium IOL upgrade (PanOptix, Vivity, Light Adjustable Lens, etc.) or femtosecond laser, those are out-of-pocket. Dr. Mudgil's team provides a written estimate at your consultation showing exactly what your final cost will be.

Does Medicare cover premium IOLs like PanOptix or Light Adjustable Lens?

No. Medicare and commercial insurance cover the standard monofocal IOL only. Premium lens upgrades and femtosecond laser fees are out-of-pocket. HSA and FSA funds may be eligible to cover some of these costs, and CareCredit financing is also available.

Can I use my HSA or FSA for cataract surgery upgrades?

Yes. Premium IOL upgrades, femtosecond laser cataract surgery, EVO ICL, contact lens fittings, and most elective vision services at Mudgil Eye Associates are HSA and FSA eligible expenses. Bring your HSA/FSA debit card or save your receipts for reimbursement.

Do you offer payment plans?

Yes, through CareCredit. Many patients use CareCredit's no-interest or extended financing options to spread out-of-pocket costs over 12, 18, or 24 months. Apply at CareCredit.com before your consultation. We also accept cash, check, and all major credit cards.

Will I get a written estimate before surgery?

Yes. For any cataract surgery, EVO ICL, or other surgical procedure, our team prepares a written estimate showing your insurance coverage and your remaining out-of-pocket cost. You will know your exact final cost before scheduling.

What about Workers Compensation eye injuries?

We accept Workers Compensation for eye injuries sustained on the job. You will need your claim number and authorization from your employer's Workers Comp carrier before your visit.

What if I don't have insurance?

We work with patients without insurance to offer transparent self-pay pricing. Many services are surprisingly affordable, and CareCredit can help you spread payments over time. Call us to discuss self-pay rates for your specific situation.
